This request form is for faculty currently seeking Tenure and/or Promotion within the next six (6) months.

Annual Faculty Evaluations

The request form will be sent to your Department Chair to generate your IDEA Faculty Summary Evaluation form. If you have any questions, please contact your Department Chair after submission.
