If you’re interested in renewing a student organization that is no longer active, you may do so by submitting the following application and attaching a constitution, a potential roster, and a brief explanation detailing why the organization should be renewed.

Please list contact information for the chief officer (President), Vice-President, Treasurer, and two general members of your organization. Please note that completing this form authorizes the Student Life Office and/or designee to verify academic and disciplinary good standing with Angelo State University.
By completing the form, you agree that you have read, understand, and shall abide by the policies outlined in the Student Handbook Part II Community Policies, Section M. Student Organizations.
Also, note that University offices and officials have the right to access any information listed on this form.
To obtain Registered Student Organization status, student groups must have at least 5 currently enrolled students included in the list below including executive officers and general members.
