The Office of Student Disability Services has varying types of learning technology, tools, and other equipment that may assist students’ with their active accommodations inside and outside the classroom. Equipment availability may vary and checkouts are on a first come, first serve basis.

I understand that I am checking out the equipment listed above for my use as a student at Angelo State University, and I am not to loan this equipment to anyone else. I understand I am responsible for the care and proper use of this equipment, and I am financially responsible for the replacement cost, if lost or damaged, due to my misuse or neglect; I will not be able to register or request a transcript until this amount is paid to the university. I understand a hold will be placed on my record preventing me from registering for classes or receiving a transcript as long as I have the equipment checked out. I will return the equipment to the Office of the Dean of Students at the end of the semester or earlier, if no longer needed.
