Blackboard Course Template or Organization Request Form

Use this form to request a new course template or organization. Course Templates are typically used as a virtual space to work on course builds or course content. If you’re preparing to teach a course in Blackboard Ultra, be sure to select the Ultra Course Template option below.

Organizations can be used for a variety of reasons. A Blackboard Organization is not necessarily academic in nature but may be used for administrative, departmental, or training purposes.

If you would like to complete a course copy after the template is requested, complete the Course Copy Request form or reach out to the Center for Digital Learning and Instruction.

Use the space below to tell us what you have planned for enrollment. You can enroll users yourself or you can attach a file with users and roles identified. Alternatively, if you want to control enrollment based on a Banner program code, you can list that code below.
